As a courtesy, we attempt to contact our
patients to remind of an appointment. We
request a call 24 hours prior to cancel or
reschedule an appointment to avoid cancellation fees.
Payment and/or co-pays are collected at the
time of service. Our practice accepts
cash, check or Visa/MasterCard and Discover.
We file claims for participation insurances and
Medicare. The patient is responsible for
any unmet deductible or
Prescriptions and Refills:
Prescription refills can be requested during
office hours. Certain medications require a
follow-up visit or lab test prior to being refilled.
It is helpful to have the telephone number of your
pharmacy available when you call.
After hours, there is a telephone recorded
message which instructs how to contact the
physician on call if an urgent problem arises.
In the event of an emergency, please call 911.
